Take advantage of the summer months to get the needed exams for your child.Īccording to law, students may be excluded from school if the physical exam and immunization information are not completed by October 15th of the current school year. No student may enter school without the proper Health Records on file. All children entering school for the first time or transferring from one school to another are required by law to have these exams. All exams must be documented on the State of Illinois forms. The State of Illinois mandates that all school age children receive physical, eye (by an optometrist or ophthalmologist) and dental exams prior to the first day of school.
